What exactly is a tip?

As many places do when there is a large group, they added the tip to the bill. Okay.
It was 20% after tax that they added - about $35.
Everyone put in their bits and handed it in without any of us totalling it first. Three or four of the people had left immediately after putting in their bits.
The waitress came back with it vehemently insisting that we were $7 short and we'd better do something about it.
This is $7 short of the total with the 20%. That means we still had a greater than 15% after tax tip in there, even more if you calculate before tax like many people do.
We weren't short on the actual bill and the tip was reasonable. She was acting like we were bad people not paying the bill. Most of them caved and they wound up putting in the other $7.
The service was barely okay to begin with and the bitchiness and demanding accusatoryness of her tone didn't help. No way was I willing to put more in after that. The others were so cowed by it, that it seemed pointless to intervene with them though.
Oh yeah, and she clarified that this was after they subtracted for the item they forgot to bring, but I'm not the least bit sure the tip (and even maybe tax) were recalculated after the subtraction.
